RESUMO

Angioplasty has taken an important place in treating cardiovascular ischemic lesions, and stenting has become a widespread routine practice. Stent loss is a complication that, although rare, can result in stent migration into a vessel with dramatic complications due to occlusion. We report the case of a 77-year-old woman who underwent surgery to remove a loose stent that had migrated into the right distal ulnar artery just before the superficial palmar arch following coronary angioplasty. The stent could be removed with the help of fluoroscopy and microscope while preserving vascular integrity. Different therapeutic strategies can be discussed in front of a stent lost in the general circulation, and those must be approached on a case by case basis.

AIMS: To compare the Escherichia coli quantification given by the 'Coliplage' assay, based on the direct measurement of the beta-D-glucuronidase (GLUase) activity and the reference Most Probable Number (MPN) method from seawater sites and investigate the possible interference of non-E. coli strains in the GLUase activity measurement. METHODS AND RESULTS: Comparison performed from 69 French coastal bathing sites (1401 samples) showed nonconcordance between both methods, only for 8% of samples. Non-E. coli 4-methylumbelliferyl-beta-D-glucuronide (MUG+) were isolated from nonconcordant samples. Phylogenetic analysis showed that Gammaproteobacteria were dominants and mainly represented by Vibrio species, which displayed GLUase activities on the same order of magnitude and sometimes much higher as E. coli reference strains. CONCLUSIONS: The'Coliplage' assay is a rapid method for the quantification of E. coli showed few discordances with the standard MPN method. Some Vibrio species could interfere on the direct GLUase activity measurement of E. coli. SIGNIFICANCE AND IMPACT OF THE STUDY: Data present the first qualitative investigation on disagreement between Coliplage and the MPN results. If the interference of Vibrio species is confirmed in situ, appropriate treatments should be developed to remove the interfering signal.

Seventy-nine recurrences have been seen after surgical procedures for recurrent dislocation of the shoulder. There were 58 "true" recurrences, 17 shoulders with anterior instability and 4 with instability in all directions. The "true" recurrences were more frequent in young people with a mean age of 20 years at the time of the first surgical procedure. Two-thirds of them occurred on the dominant side. Most of the cases had been treated by distal displacement or lengthening of the coracoid process (38 Trillat procedures, 8 Latarjet procedures and 8 Oudart procedures). Only two had been treated by a Bankart procedure. The main cause of recurrence was a failure of repair of the antero-inferior part of the joint. Twenty three cases were operated on again, the most usual procedures being the Trillat procedure and rarely the Latarjet or Eden Hybinette procedures. Six cases recurred once again. Cases with anterior instability were observed after the Trillat procedure. They were due to capsular laxity and did not have to be operated on again. Instability in multiple directions was observed in young women who continued to dislocate their shoulder inferiorly or posteriorly, despite two or three surgical procedures.

SUMMARY: A factor of protein nature that forms specific complexes with low density lipoproteins (LDL) has been detected in extracts of aortic intima-media from men and rabbits. Although the formation of complexes is maximal at low ionic strength, it is still observed under physiological conditions. Gel electrophoresis of the complex formed between the extracts and LDL proteins indicates that the insoluble material is an aggregate of the lipoproteins with some of the proteins present in the extracts. Pretreatment of the intima-media extract with proteases greatly diminishes its complexing ability. The interaction was also observed between the extracts and sera. The sera from patients with myocardial infarct formed more insoluble complexes than the sera from a control group. The lipoprotein complexing agent was found in intima-media of aorta and coronary arteries but not in that of veins, pulmonary arteries or in the adventitia of arteries or veins.
